The next step is completion of paperwork. As with any other physician or personal care provider, there are forms that must be completed prior to the first visit which facilitate smooth service on the behalf of your provider's office. To assist the staff and expedite service, you can download these forms at your convenience and then bring the completed paperwork to your first appointment. These forms can be found under the New Patient tab on the Forms page.
If you are filing insurance, please bring your insurance card and ID so we may obtain a copy.
Cancellation Policy - The policy states that any changes to scheduled appointments should be made at least 24 hours in advance. Missed appointments which are not properly cancelled are subject to an missed appointment fee, which can be equal to but not exceed the therapist's regular appointment fee. Insurance companies do not pay missed appointment fees and the patient/responsible party is held fully accountable for the charge.
